- Fix iconv_mime_decode() to correctly handle "malformed" headers :)
- Fix iconv_mime_decode() bug, that the result is mangled when
ICONV_MIME_CONTINUE_ON_ERROR is enabled and the function encounters an
unconvertable encoded chunk.
