Buncha Fixes.
...
* Fixed a number of memory leaks.
* Fixed some php_error_docref() calls that tried to print non-existent
arguments.
* Fixed some signed/unsigned problems.
* Fixed the MYSQLI_FETCH_RESOURCE macro so that compilers do not complain
about un-initialized variables.
* CS fixes.
