Require that wrappers use <protocol>://<path> to avoid

ambiguities when filenames have ':' characters.
This slightly breaks BC with the old style zlib: wrapper.

dup fds before fdopen'ing them, so that people cannot deliberately

close stdio streams.

fopen wrappers cleanup
...
- comfiguration is now done by an ini parameter
instead of a compile time option
- the implementations of the three standard wrappers
now live in seperate files in ext/standard
- the compiler is happy again, no more warnings
