06f6e47e4c Require that wrappers use <protocol>://<path> to avoid
ambiguities when filenames have ':' characters.
This slightly breaks BC with the old style zlib: wrapper.

leaking streams should be easier.

6558ae6f21 dup fds before fdopen'ing them, so that people cannot deliberately
close stdio streams.

This needs to be tested on non-UNIX platforms.

cae27179ce fopen wrappers cleanup
- comfiguration is now done by an ini parameter
    instead of a compile time option
  - the implementations of the three standard wrappers
    now live in seperate files in ext/standard
  - the compiler is happy again, no more warnings
